Chronic medication conciliation to preoperative management by electronic prescription

Abstract

Our aim is to adjust the chronic treatment of patients with scheduled surgery to the hospitals in a single electronic prescription. The medication conciliation program in surgery is described by electronic prescription including the preoperative medication. Conciliation begins by the pharmacist's treatment recommendation to make it available on preoperative visit and may be validated by the anaesthetist and/or surgeon. During the study period, the medication was conciliated in 938 patients and 2,733 drugs. The physician accepted 60.34% of the recommendations. In addition, anesthesiologists and surgeons added to these orders their proper preoperative treatment protocols. The physician modified the pharmacist recommendation in the following cases: changes by medication adjustment to hospital protocols or treatment updates (dosage, new drugs).
