This article discusses the importance of pain neuroscience education and therapeutic exercise as non-pharmacological treatments for persistent pain. It emphasizes the need for cognitive changes, such as modifying beliefs, to reduce catastrophizing and kinesiophobia. Education in pain neuroscience helps patients understand that persistent pain does not always indicate tissue damage. The article also highlights the role of the central nervous system in maintaining and perpetuating persistent pain. Therapeutic physical exercise is fundamental in the treatment of persistent pain as it reduces central sensitization and improves physical function. It recommends creating a therapeutic alliance, fostering positive expectations, addressing false beliefs, providing education on the neuroscience of pain, and promoting therapeutic exercise. However, implementing these recommendations in clinical practice presents challenges, and more high-quality clinical trials are needed.
